Introducing Our Handcrafted Leather Pouch for the Twiist Insulin Pump
When the Twiist insulin pump launched in July 2025, we knew right away it would be one of the more interesting design challenges we've taken on. Almost every insulin pump we've made a pouch for over the years — the Tandem t:slim X2, the Medtronic 780G, the iLet Bionic Pancreas — has been rectangular. Twiist is round.
That single detail changes everything about how a pouch is built.
The Challenge of a Round Pump
At EXODOS Copenhagen, every pouch starts as a wooden pattern, hand-shaped by George to the exact dimensions of the device it's made for. It's what allows us to wet-mold leather into a case that holds its form for years rather than months — and it's the same approach behind every model we make, from the Tandem t:slim X2 to the iLet Bionic Pancreas.
Twiist, though, was a different kind of challenge. Almost every pump we've built a pattern for has been rectangular; Twiist is round. A round device doesn't work with a rectangular pattern scaled down — the curves, the depth, and the way the pump sits against the body all have to be worked out from scratch. So George carved a dedicated round wooden pattern for it, shaping the curve by hand until the fit was right. It's a small thing most people will never think about, but it's the difference between a pouch that fits like it was made for the device, and one that was just "close enough."
Built the Same Way We Build Everything
Once the pattern existed, the rest followed our usual process:
- 7oz (2.7mm) full-grain, vegetable-tanned Italian cowhide — thick enough to protect the pump, supple enough to shape.
- Traditional wet-molding — the leather is shaped wet around the pattern and dries into a rigid shell that keeps its shape permanently. No sagging, no softening, no reshaping itself around your hip after a few months like fabric or synthetic pouches tend to.
- Hand-stitched construction for durability and a clean, refined finish.
- A small push-up access hole, so you can see and reach the pump's screen without removing the pouch.
- A velvet-lined snap closure — the metal snap sits against soft velvet on the inside, so it never scratches the device.
- Choice of belt loop or clip, depending on how you like to carry it.
- Four colorways — black, brown with beige stitching, brown with brown stitching, or natural undyed leather that develops a beautiful patina over time.

A Design That Happens to Hold a Medical Device
A lot of the pouches available for insulin pumps look exactly like what they are — clinical, plasticky, hard to miss. Ours are built the same way we build all our leather goods: inspired by Danish design principles of function, quality, and understated elegance. People tell us they've had others compliment the pouch without realizing it's holding a medical device at all — and that they'll switch colors between a natural tone for the office and black for evenings out, the same way they'd choose a belt or a bag.

We launched this pouch alongside a new pouch for the Tandem Mobi, and it's quickly become one of our best performers — customers using Twiist have been ordering it steadily since launch, both through our shop and through our partners at The Useless Pancreas in the US.
